Molto di rado la nascita di una nuova idea o di una nuova organizzazione è la conseguenza di un evento singolare. Eppure, la nascita di questa nuova professione, la Chiropratica, ha una data e un luogo ben precisi: il 18 settembre 1895 a Davenport, nell’Iowa.
Daniel David Palmer pose le mani su di una protrusione irregolare della colonna vertebrale di Harvey Lillard e con una poderosa spinta ne ridusse l’irregolarità. Dopo l’intervento di Palmer, Mr. Lillard affermò di poter “sentire i carri nella strada”, cosa che non poteva fare prima del trattamento (Palmer, 1910).
Sul finire del XIX secolo, nell’America contadina la medicina era più una forma d’artigianato che un’arte. L’integrazione della scienza nei metodi terapeutici e nella pratica medica era decisamente carente, come testimoniato dalla severa condanna delle facoltà di medicina contenuta nel noto Rapporto Flexner (Flexner, 1910). Il consolidamento della “autorità culturale” (Starr, 1982) dei medici allopatici non era ancora compiuto, e c’era la concorrenza di numerosi magnetoterapisti, erbalisti, guaritori, conciaossa e omeopati.
La crescita delle medicine alternative andava di pari passo con il revivalismo nelle pratiche religiose e si riteneva fosse la controparte fisiologica al perfezionismo teologico del tempo (Fuller, 1989). Questo crogiolo confusionario, pieno di vitalismo e magnetismo, sanguisughe e bisturi, tinture e cataplasmi, era il terreno fertile per il pensiero creativo e la nascita di nuove idee. D.D. Palmer e la Chiropratica erano, almeno in parte, un prodotto del loro tempo. Nell’America d’inizio secolo, i medici allopatici guadagnarono quella superiore “autorità culturale”, insieme al rispetto di coloro che influenzavano i processi decisionali. L’opposizione alle medicine alternative crebbe di conseguenza. Il medico allopatico accusava il dottore in Chiropratica di praticare l’attività medica senza la necessaria licenza. Il dottore in Chiropratica ribatteva che la Chiropratica e la pratica medica erano cose assai distinte.
Per sottolineare la differenza, la comunità chiropratica sviluppò per il proprio metodo un lessico e principi a essa peculiari (Keating, 1989). Il metodo della medicina, con la ricerca del processo patologico, la relativa classificazione e la prescrizione della cura era diverso dalla ricerca chiropratica di un’interferenza nel sistema nervoso che fosse sia causa remota, che non immediata, di una disfunzione e della malattia. Il dottore in Chiropratica rifiutava usare medicamenti e farmaci e non adottò mai la pratica chirurgica. La Chiropratica era concepita come un metodo di cura naturale, che attingeva alle forze di recupero del corpo stesso.
Nonostante il conflitto caratterizzasse buona parte dei rapporti tra la medicina istituzionale e la Chiropratica, questa polarizzazione riguardava soprattutto questioni economiche, politiche e giuridiche piuttosto che i rapporti clinici. Basti pensare che D.D. Palmer attribuiva a un medico, Jim Atkinson, il merito di avergli insegnato le tecniche di manipolazione delle ossa in altre culture (Palmer, 1910, p. 789).
G.H. Patchin, MD, era il medico che aiutò Palmer nella stesura del suo libro, The Chiropractic Adjuster, e non possiamo ignorare che un terzo della prima classe di laureati in Chiropratica erano medici (Palmer, 1910; Gibbons, 1981).
Dopo la pubblicazione del Rapporto Flexner (1910), la disciplina medica consolidò e rinsaldò la propria posizione nella società. In tal modo, sia l’insegnamento che la ricerca medica ottennero il sostegno finanziario del governo federale e di fondazioni private. In un primo momento, i fondi federali furono destinati al finanziamento dell’assistenza medica ai veterani di guerra, e successivamente agli anziani e ai disabili.
Al contrario, l’insegnamento della Chiropratica restava un’attività a pagamento, con scarsi mezzi e senza aiuti esterni per la ricerca. Nel tentativo di sopprimere la Chiropratica, la medicina ufficiale incoraggiò la regolamentazione della pratica medica, immaginando che il minore livello d’istruzione conseguito nelle scuole di Chiropratica avrebbe impedito a coloro che vi si erano laureati di superare gli State Board Licensing Exams, gli esami di abilitazione (Gevitz, 1988; Wardwell, 1992). L’introduzione dei Basic Science Boards, le commissioni esaminatrici per le materie scientifiche di base, sostenuta dalla professione medica nel 1925, rappresentava un ulteriore ostacolo per il laureato in Chiropratica, proprio per l’assenza di tali materie nei loro programmi di studi. Ma le scuole di Chiropratica reagirono: potenziarono le proprie strutture educative, ampliando i programmi di studi e impiegando docenti di livello universitario per l’insegnamento delle materie scientifiche di base. I chiropratici iniziarono così a superare gli esami dei Basic Science Boards.
L’impegno al miglioramento della qualità delle proprie strutture educative, condusse infine alla creazione di un ente specifico per il riconoscimento delle facoltà di Chiropratica, il Council on Chiropractic Education (CCE), che nel 1974 ottenne il riconoscimento federale dal Ministero dell’Istruzione. Fu quest’istituzione a definire gli standard minimi essenziali sia per i programmi di studi sia per le norme di ammissione. Le scuole che non furono in grado di adeguarsi agli standard del CCE, furono costrette a chiudere.
Nel 1995, tutti i college di Chiropratica avevano ottenuto il riconoscimento del CCE. Proprio come aveva fatto il Rapporto Flexner sulle università di medicina, il CCE produsse un innalzamento della qualità dell’insegnamento in molte scuole di Chiropratica. Fino a tempi recenti, la Chiropratica è stata accusata dalla medicina allopatica di essere un culto ascientifico, non corredato da studi e ricerche a sostegno della propria efficacia (Keating, 1993; Wardwell, 1992).
La ricerca era effettivamente stata trascurata durante i primi anni di vita della professione. Senza finanziamenti per la ricerca e per le strutture degli istituti di insegnamento a pagamento, le limitate risorse dei primi college erano concentrate sulla qualità dell’insegnamento, piuttosto che sulla creazione delle basi scientifiche della professione. Poco a poco però, iniziarono a vedersi segni incoraggianti: Watkins, Weiant, Higley, Illi, e Janse, tra gli altri, trovarono la soluzione degli inspiegati successi del trattamento e compresero come una solida base di ricerca fosse l’arma da usare per confutare le accuse degli avversari. La creazione della Foundation for Chiropractic Education and Research (FCER) ha dato l’impulso necessario a questa cultura della ricerca. Oltre a sostenere i lavori di ricerca, la fondazione, ha avviato nel 1977 un programma di finanziamento per la formazione di ricercatori chiropratici.
Ormai esiste un consistente gruppo di pensatori critici nella professione e un crescente numero di ricercatori esterni alla professione impegnati nello studio della Chiropratica. Nel 1996, i fondi del governo federale per la ricerca sono stati concessi a quattro college di Chiropratica. In questi ultimi anni, è inoltre cresciuta la collaborazione tra i chiropratici e la comunità scientifica e clinica sui temi della formazione, della ricerca e della pratica clinica (Mootz, 1995).
Ormai comuni sono gli studi polispecialistici, così come le collaborazioni editoriali e tecniche, i programmi di ricerca comuni e l’appoggio di dottori in medicina a chiropratici nelle vertenze giudiziarie (Mootz, 1995). Ora che la professione è sempre più coinvolta in studi critici e concentrata sul proprio perfezionamento, è ancora più difficile applicare alla Chiropratica la definizione di culto ascientifico.
La ricerca ha dimostrato che la manipolazione, uno dei principali metodi terapeutici del dottore chiropratico, è efficace nel trattamento delle lombalgie acute (Shekelle, 1992). La menzione della manipolazione tra i trattamenti raccomandati dalle linee guida delle autorità federali per la cura delle lombalgie acute, altro non è che il risultato degli studi di ricercatori sia esterni che interni alla Chiropratica (Bigos, 1994). La ricerca, ormai impegnata nella valutazione del ricorso al trattamento chiropratico anche per altre situazioni cliniche, svelerà le potenzialità e anche i limiti della Chiropratica, stimolando rapporti interdisciplinari che miglioreranno il trattamento generale del paziente.
Ci sono voluti 100 anni di sforzi autonomi, con il solo aiuto del finanziamento interno, per portare la Chiropratica nei sistemi sanitari generali. Con l’inserimento nel mondo della sanità ufficiale, le questioni del ruolo e degli scopi della prassi chiropratica sono ormai d’estremo interesse.
La Chiropratica è un’alternativa alla medicina? È possibile pensare a un ruolo complementare, inserito in un sistema sanitario collaborativo? La Chiropratica dovrebbe rimanere una professione separata e distinta oppure dovrebbe cercare di essere inglobata nella medicina come sottospecializzazione nelle affezioni muscoloscheletriche. L’insegnamento della Chiropratica dovrebbe cercare di appoggiarsi alle grandi università che ospitano corsi di medicina? Le risposte a queste domande forniranno le direttive per gli sviluppi futuri dell’esercizio e dell’insegnamento della Chiropratica.
